Before your first visit, we invite you to complete your new patient forms from the comfort of home. These documents can be cumbersome and time consuming to complete in our office, but by downloading and filling these documents at home, we hope to save patients time in our office.

Today was my first visit to Dr Cooke's office. His hygienist took full-mouth x-Rays, which Dr. Cooke took time to review with me, while he examined my teeth. I was shocked to learn that my teeth had been badly damaged by the side effects of immunotherapy. So much so that I was impressed when Dr Cooke indicated he would consult with one or more of his colleagues to get their opinions and contact my oncologist to coordinate treatment options.
